RP Home has been providing high quality care to residents for decades. We offer a supportive environment for residents who require post-acute, skilled and intermediate nursing services, as well as short-term or comprehensive rehabilitation.
Our goal is to help you achieve your maximum level of physical, social, mental, and spiritual well-being. Our dedicated professionals are here to assist with person-directed care.
Whether you need round-the-clock help or short-term care to recover from surgery or injury, we’re here for you. Our expert care includes:
- Interdisciplinary Care Plan Teams that develop individualized resident care plans in coordination with the resident and their family

- Hospice Services in coordination with a licensed hospice provider

Our dedicated staff provides group and individual activities. You will have the choice to attend a variety of activities such as musical programs, movies, games, socials, worship and intergenerational activities.
Several public spaces are available for relaxation and socialization such as the Chapel, Linton Foyer, the Library and the Garden of Faith Courtyard.
Rehabilitation
RP Home partners with Premier Therapy to provide innovative care for both short-term and long-term residents. Premier Therapy has a highly-trained team of managers, therapists, systems and signature programs to provide positive outcomes.
Through Physical, Occupational, and Speech Therapy, we provide a wide variety of activities to stimulate your interests and ability to heal. We work with you at your own pace so that you get back to a life of independence as quickly and safely as possible.
Our Physical Therapists work with individuals to improve mobility, ambulation and transfers. In addition, we will provide modality treatments such as moist heat treatment, ultrasound, massage and electrical stimulation to improve functioning. Our therapists will provide an individualized therapeutic exercise program to improve strength, endurance, range of motion stretching, and balance/coordination.
The major focus of Occupational Therapy is re-training individuals with injuries or debilitating conditions in Activities of Daily Living, such as dressing, bathing, and homemaking. In addition, Occupational Therapy will provide an individualized exercise program; static/dynamic splinting program; and a cognitive re-training program. Adaptive equipment and other environmental adaptions will be evaluated to ensure each person reaches his or her maximum independence.
The Speech Pathologist (SP) assesses and treats individuals who experience communication and swallowing difficulties. S/he will evaluate the individual’s speech, language/communication and cognitive/thinking/reasoning skills to structure a meaningful therapy program to enhance or restore the highest possible level of function and ability. The Speech Pathologist is certified to provide VitalStim Dysphagia Therapy. In addition, our SP is a Certified Dementia Practitioner; Certified Montessori Dementia Practitioner; and a Board Certified Specialist in Swallowing and Swallowing Disorders by the American Board of Swallowing and Swallowing Disorders.
SPEAK OUT!®
For residents with Parkinson’s Disease
The Parkinson Voice Project has developed an effective program that combines education, individual speech therapy (SPEAK OUT!), and ongoing group sessions (The LOUD Crowd®). Together, the patient and speech-language pathologist complete speech, voice and cognitive exercises using a specialized workbook that the Parkinson Voice Project provides. SPEAK OUT! emphasizes speaking with intent and converts speech from an automatic function to an intentional act.

Wound Care
RP Home provides a team of professionals to coordinate your wound care program. A Certified Registered Nurse Practioner (CRNP) who is certified in wound care leads the team. The wound care team also includes: a wound care nurse, dietician and a member from of the therapy team. All work together to develop a care plan that addresses the modalities necessary to treat, heal and prevent wounds.
Post Discharge Support
We are committed to ensuring your health continues after completing your rehab program. Members of our team will remain in contact with you for 45 days to monitor your progress, safety, and answer questions. Our hope is to prevent avoidable circumstances which could lead to a re-hospitalization.
